For detailed information as to how to obtain certified copies of various evidential documents, please visit the CDC website directly.

Please note all documents must be certified copies or original and you cannot obtain a certified copy of your drivers license.

You can either send the original you have or get more certified copies from the place that initially issued the certificate. In other words, you will have to get extra "copies" of the "original" from the issuing authority, therefore getting multiple "originals".

Only official, original documents are accepted as they must meet the following requirements:

  • All documents must be either originals or certified copies by the issuing agency (certified copies may be obtained from the agency that issued the original certificate or document)
  • Photocopies, notarized copies, or receipts showing you applied for the document are NOT accepted
  • Documents must be current, expired documents are NOT valid
  • Documents should be issued from the US Government
  • Birth Certificates are not acceptable as proof of identity

Depending on your case, you may be asked to provide one proof of identity and may also be asked to provide one proof of age.

Please note that you will first receive your new social security card and then you will receive your supporting documents a few days after.

Depending on your case, you may have to provide proof of citizenship, proof age and/or proof of identity.

For proof of citizenship, the SSA can accept only certain documents such as a U.S. birth certificate or a U.S. passport.

For proof of age, you must present your birth certificate. (If one exists, you must submit it.) If a birth certificate does not exist, the SSA may be able to accept your:

  • Religious record made before the age of 5 showing your date of birth
  • U.S. hospital record of your birth
  • U.S. passport

For proof of Identity, the SSA will accept the following:

  • U.S. driver’s license
  • State-issued nondriver identification card
  • Health insurance card (not a Medicare card)
  • Employer identification card
  • Military identification card
  • School identification card
  • U.S. passport
  • Certified copy of medical record

Please note that the SSA can use one document for two purposes. For example, you can use your U.S. passport as proof of both citizenship and identity. However, you must provide at least two separate documents.

To report identity theft, fraud, or misuse of your Social Security number, the Federal Trade Commission (FTC), the nation's consumer protection agency, recommends that you take the following steps:

  • Step 1: Place a fraud alert on your credit file by contacting one of the three companies:
    • Equifax - 1-800-525-6285
    • Trans Union - 1-800-680-7289
    • Experian - 1-888-397-3742
The specific company you contact is required to contact the remaining two, which will then place an alert on your credit report.
  • Step 2: Review your credit report for inquiries from companies you have not contacted; accounts you did not open; and debts on your accounts that you cannot explain.
  • Step 3: Close any accounts you know, or believe, have been tampered with or opened fraudulently.
  • Step 4: File a report with your local police or the police in the community where the identity theft took place.
  • Step 5: File a complaint with the Federal Trade Commission online or by calling them at 1-877-438-4338 (TTY 1-866-653-4261).
